New Students
The following documents are required to complete the application process:
· An official document showing proof of birth date (birth certificate, certified birth record, official transcript).
**Must be age 5 by September 1st to start kindergarten.**
· Current proof of residency (mortgage, lease, or utility bill showing name of parent/guardian and current address)
· Valid Georgia Certificate of Immunization (GA Form #3231)
Evidence of Vision, Hearing, Dental, and Nutrition Screening (GA Form #3300-Rev. 2013)
· Valid government-issued photo ID of the adult (parent or legal guardian) registering the child (drivers license, military ID, passport)
· Child’s social security card/number (or you may sign a waiver)
· Most recent report card or official school transcript (current K-12 student)
· Most recent discipline records from previous school (students enrolling in grades 1-12)
· Withdrawal documents from previous school (current K-12 student)
DEKALB COUNTY SCHOOL DISTRICT CURRENTLY ENROLLED STUDENTS
Please visit the following website and follow the guidelines of CURRENTLY ENROLLED Students:
· Parent/Guardian must have an Infinite Campus Parent Portal account to verify/update their student(s) information.
· Parents/guardians who do not already have a portal account should visit Infinite Campus for instructions.
Parents/guardians who have forgotten their portal username and/or password should:

You will need to Upload the following documents: Parent/Guardian ID and Proof of Residence (current gas, light, or water bill). You may also choose to upload a lease with the company logo, current address, contact information, and parent/guardian name listed. Only the page(s) with this information has to be uploaded. A full lease is not needed.
